This program offers non-medical personal care and respite services to assist individuals with physical disabilities in remaining independent at home, avoiding the need for nursing home care. Language support is available in English, Spanish, and Tagalog, along with translation services.
Intake Procedure: You can apply by walking in, calling, emailing, or faxing; the CBC application can be found on the website.
Fees: There are no fees for those who meet income guidelines; otherwise, a co-pay is required.
