About this resource
This initiative provides airline tickets for those needing to travel long distances for specialized medical care and second opinions. The flights are directed to treatment facilities within the United States and also accommodate trips to retrieve service dogs or attend necessary training sessions. Eligible recipients may travel with up to two parents or legal guardians and can receive assistance as frequently as required.

Who can use this
- -Ages 0-17 -Must adhere to income guidelines; contact for specifics -14 days advance notice needed from the desired departure date
Documents you may need
- -Application package including specific flight request -Local physician referral letter -Medical appointment confirmation letter -Proof of income (gross annual household income from all sources combined) with the first two pages of the most current 1040 federal tax return and/or SSI / SSDI statements -Birth certificate -Domestic Program: All documents must be received at least 14 days prior to requested departure date
