About this resource
The service provides comprehensive primary care for seniors, featuring cardiology telehealth consultations, nutritional guidance, laboratory testing, preventive health screenings, routine blood tests, and tests for influenza A and B, COVID-19, and strep throat. It also includes skin cancer checks, urinary incontinence evaluations, and vaccinations for flu, shingles, tetanus, and pneumonia. Same-day medical appointments may be available. Services are offered in both Spanish and English.

Who can use this
- Eligibility is for seniors aged 60 and above who are enrolled in Medicare Advantage Plans or D-SNP Medicaid plans.
