About this resource
This program provides primary care services tailored for seniors, featuring telehealth cardiology consultations, nutritional support, laboratory testing, preventive health screenings, routine blood tests, and tests for influenza types A and B, COVID-19, and strep throat. It also includes skin cancer screenings, assessments for urinary incontinence, and vaccinations against Flu, Shingles, Tetanus, and pneumonia. Services are available in both Spanish and English.

Who can use this
- Eligible participants are seniors aged 60 and above who are enrolled in Medicare Advantage Plans or D-SNP Medicaid plans.
