This program provides primary care services tailored for seniors, featuring telehealth cardiology consultations, nutritional guidance, laboratory testing, preventive health screenings, routine blood tests, and assessments for influenza types A and B, COVID-19, strep throat, skin cancer, urinary incontinence, along with vaccinations for flu, shingles, tetanus, and pneumonia. Same-day medical appointments may be available. Services are offered in both Spanish and English.

Who can use this
- Eligible individuals are seniors aged 60 and above enrolled in Medicare Advantage or D-SNP Medicaid plans.
