This program provides primary care services tailored for seniors, featuring telehealth cardiology consultations, nutritional advice, laboratory testing, preventive health screenings, routine blood tests, and tests for influenza A and B, COVID-19, and strep throat. It also includes skin cancer checks, urinary incontinence evaluations, and vaccinations for flu, shingles, tetanus, and pneumonia. Same-day medical appointments may be available. Services are offered in Spanish, Vietnamese, and English.

Who can use this
- Seniors aged 60 and above who are enrolled in Medicare Advantage Plans or D-SNP Medicaid plans.
