About this resource
This program coordinates community resources and services to assist eligible individuals residing in long-term care facilities, rehabilitation centers, or hospitals who are at risk of being placed in long-term care but wish to stay in the community. Additionally, translation services are available.
Intake Process: Individuals can walk in, or they may call, email, fax, or check the website for further information. A FOCIS referral can also be found on the website.
Cost: There are no fees associated with this service.

- Individuals in nursing homes, rehabilitation centers, or hospitals at risk of long-term care placement.
