About this resource
This Federally Qualified Health Center delivers primary preventive care services for children and adults, such as annual physical examinations, consultations for minor ailments, well-child check-ups, dental evaluations, and health education. It also offers medical management for those with chronic health issues. Telehealth options are available for established patients of SNCHC. Services are provided in both Spanish and English.

Fees: A sliding scale is used for payment.
