About this resource
This program provides support to older adults, individuals with disabilities, the Deaf and Hard of Hearing community, caregivers, and families by helping them identify their needs and goals while exploring long-term services and support options. It includes assistance with applications, determining eligibility for public programs, and caregiver support, among other services. Available languages are Spanish and English.

Fees: No charge.
Who can use this
- Priority is given to seniors aged 60 and above and individuals with disabilities, but services are accessible to all, regardless of age or income, to utilize community long-term services and supports.
