Comprehensive case management aids seniors in formulating a personalized care plan that addresses their physical, psychological, social, and financial requirements. Case Managers are responsible for coordinating necessary services and monitoring cases over the long term to guarantee that seniors obtain the services they require. Additionally, Case Management services include help with completing paperwork for various programs. Languages available: English.
