Lyon County Human Services

This program delivers support and case management to individuals classified as homeless or at risk of homelessness who are dealing with serious mental health conditions or physical illnesses that hinder their ability to work. Services encompass the entire social security application process, including assistance with applying, monitoring, and acting as the authorized representative. Additional offerings include help with community referrals, scheduling appointments, transportation support, interactive case management, opportunities for volunteering, and resources for housing and income stabilization. Translation services are available, primarily in English.
