About this resource
Delivers hospice care to patients and their families facing the advanced and final phases of a terminal illness. Services include nursing visits, 24/7 support, provision of medical equipment and supplies, home delivery of medications, Medical Social Services, Certified Nursing Assistants, Spiritual Care, Bereavement Support, short-term acute inpatient care/respite care, as well as physical, occupational, and speech therapy. Language support is available through translation services in Spanish and English.

Fees: Charged at a daily rate.
Who can use this
- Must be certified by a physician as terminally ill with a prognosis of six months or less and should not be pursuing curative treatment, except for children who may continue to seek curative options.
