About this resource
This program facilitates the coordination of community resources and services for eligible individuals residing in long-term care facilities, rehabilitation centers, or hospitals, who are at risk of needing long-term care but wish to stay in the community. Translation services are available.
Intake Process: Individuals can walk in, or they may call, email, fax, or visit the website for more information. A FOCIS referral can be accessed on the website.
Cost: There are no fees associated with this service.

- Individuals in nursing homes, rehabilitation centers, or hospitals at risk of long-term care placement.
