About this resource
Offers assistance to individuals diagnosed with HIV/AIDS or other chronic conditions in accessing housing, medical care, and medication, as well as social services related to food and transportation arrangements.
Intake Procedure: An appointment is necessary; please call for details.
Fees: There are no charges.
Who can use this
- To qualify, individuals must: 1) Have active Medicaid 2) Be living with two or more chronic health issues, OR have HIV/AIDS, OR have a significant mental illness.
