About this resource
The Aging Out Adolescent Program collaborates with the Albany County Department of Mental Health to support adolescents who are transitioning from the Children's Mental Health System to adult services, aiming for greater independence. Besides providing counseling and case management, the program also offers assistance with housing, education, vocational training, and job placement.
