About this resource
Located at the Capital District Psychiatric Center, the Community Transition Team partners with hospital personnel and the Albany County Department of Mental Health to facilitate discharge planning for patients. We aid both hospital staff and patients in selecting the best residential choices within the community, support patients during their transition back into the community, and ensure that necessary assistance is available when required.
