About this resource
Children's Health Homes was created to serve as a Health Home for Children, which is a care management model where all caregivers collaborate and exchange necessary information to promote health and recovery. This approach emphasizes family strengths and guarantees that families have a say in every aspect of the care management process. CHHUNY aims to deliver high-quality, family-centered care management for youth with health needs, facilitated by partners skilled in supporting children and their families. A care manager is primarily responsible for coordinating and providing access to all required services to maintain the child's health.

Who can use this
- Eligibility includes children and youth from birth to 21 years who are enrolled in Medicaid and have at least two chronic conditions, a diagnosed Severe Mental Illness (SMI) including Severe Emotional Disturbance (SED), or HIV, with potential future criteria for trauma and risk of another chronic condition pending CMS approval. Service Area: Allegany & Cattaraugus Counties.
