About this resource
Care Management services provide monitoring, advocacy, and connections to other resources through weekly interactions with clients. These services cater to both adults and children, and Care Managers may also assist with funding for urgent care and necessities.
Intake Process: Referrals can come from various sources such as agencies, schools, individual providers, and self-referrals. For a referral packet, please call us.
Fees: Services are covered through Medicaid.
Who can use this
- Eligibility is available for children and youth with active Medicaid who meet specific criteria (two chronic conditions, SED, or complex trauma), as well as adults with active Medicaid who qualify (two chronic conditions or SPMI). Services are offered in Cattaraugus County.
