This program offers care management services to adults who are Medicaid recipients and have a serious mental illness (SMI), HIV/AIDS, or two chronic health conditions.
Intake Process: Individuals can either walk in or make a phone call to access services.
Cost: There are no fees associated with these services.
Who can use this
- Individuals aged 18 and above who are Medicaid recipients and have a serious mental illness (SMI), HIV/AIDS, or two chronic health conditions are eligible, provided they have a Record of Qualifying Diagnosis, within Chautauqua County.
