This service provides primary care, podiatry, and school-based health services to the community; dental services are listed separately.
Intake Process: New patients must first complete an intake packet. Applications can be picked up on-site, requested by phone for mail delivery, or submitted online.
Fees: To confirm coverage, please call with your insurance details. Medicaid and Medicare are accepted. There are also options for payment plans and sliding fee scales.
Who can use this
- Open to all community members, with specialized services for individuals with developmental disabilities and the I/DD population. Primary Care & Podiatry is available for those aged 11 and older, and School-Based Health services are offered at Jamestown High School and Washington Middle School within Chautauqua County.
