About this resource
A Care Manager partners with your child and family to leverage your strengths and cultivate skills aimed at addressing any concerns regarding your child's well-being. These managers engage with the child, family, and all relevant caregivers (such as pediatricians, dentists, counselors, and school personnel) to formulate a care plan that aligns everyone towards a shared objective. If further assistance is required, your Care Manager will facilitate connections to suitable services to fulfill your child's needs. Care Management services prioritize family involvement, youth guidance, and a person-centered approach tailored to each child's specific circumstances.

Fees: There are no charges.
Who can use this
- Eligible youth are aged 0-21, Medicaid recipients, and diagnosed with Serious Emotional Disturbance, Chronic Health Conditions, HIV, or Complex Trauma, within the WNY service area.
