Erie County Adult Single Point Of Access (Spoa)

Care Coordination/Case Management Services are designed for individuals with high needs who face challenges in accessing mental health services and other critical supports, including benefits, housing, advocacy, health care, peer services, or vocational assistance. The SPOA serves as a standardized referral system that helps the referring entity identify the suitable level of care coordination services. Once a referral is submitted, it undergoes an assessment to decide whether to accept the initial referral or redirect it to a more appropriate program that aligns with the individual's needs. Available levels of care comprise transition case management, care coordination, assertive community treatment, and assisted outpatient treatment.

Intake Procedure: The referring agency must fill out the SPOA referral forms (accessible on the website) and send them to the designated agency.

Fees: There are no charges for these services.
