Expanded In-Home Services For The Elderly (Eisep) - Erie County Department For The Aging

This program provides a range of services, including information and referrals for all available services, assessments of long-term care needs for seniors, and the creation of tailored care plans. It also offers case management to connect clients with necessary services, guidance for clients and their families, and home health services. Additionally, adult day care is available to eligible clients as funding allows, with potential cost-sharing options for home health and adult day care services. A Caregiver Resource Center has been established to support caregivers of seniors and those caring for children under 18.
