About this resource
This initiative provides a range of services such as independent living skills training, information and referral, advocacy, peer counseling, transitional services, and guidance on benefits and entitlements. It also includes educational advocacy, assistance with transitioning from nursing homes to community living, individual support services, Olmsted housing subsidies, and support groups specifically for individuals with disabilities. Transportation services are available, and a loan closet offers free, gently used medical equipment like wheelchairs, walkers, and more, subject to availability.
