At the Breckenridge Street facility, the services provided include: Routine Individual and Family Care, Women's Health and Maternity/OB/GYN Care, Children's Health, Specialized Diabetes Care, Behavioral Health, Pharmacy Services that encompass Medication Management and Education, Medical Case Management and Care Coordination for complex cases, and assistance with health insurance and benefits applications.
Intake Procedure: To schedule an appointment, please call; new patients are currently being accepted.
