Neighborhood Health Center - Northwest

This Federally Qualified Health Center (FQHC) delivers comprehensive services such as pediatrics, internal and family medicine, OB/GYN, dental care for both children and adults, podiatry, social work, nutritional counseling, medications for opioid use disorder, and pharmacy services with home delivery. It caters to all demographics, particularly focusing on the medically under-insured, uninsured, and individuals facing homelessness. Additional services include onsite lab work, ultrasounds, STD testing, COVID-19 testing and vaccinations, health education groups, blood pressure screenings, and free walk-in pregnancy tests. The center also offers transportation assistance for clients to attend appointments and provides referral support. It participates in the 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations include Sisters Hospital and the John R. Intake Procedure: To schedule an appointment, please call. New patients are welcome. Phone lines are open Monday through Thursday from 8:30 AM to 6 PM and Friday from 8:30 AM to 4:15 PM.

Fees: The center accepts most insurance plans, including Medicaid and Medicare, and uninsured patients can apply for sliding scale discounts based on their income and household size. Information and counseling services are provided at no cost.
