About this resource
Group Homes offer essential services such as room and board, supportive counseling, personalized service planning, medication oversight, behavioral interventions, case management, social activities, transportation, crisis support, training for daily living skills, discharge planning, and follow-up care. The Apartment Program provides a supported housing option with adaptable support services. There are scattered site apartments available as well. Group home locations include: Wende Community Residence at 20 Wende Street, Buffalo, NY 14211, phone 716-896-2768; Fruitbelt Towers (accessible for handicapped) at 233 Lemon Street, Buffalo, NY 14204, phone 716-881-1196; Monroe Community Residence at 423 Monroe Street, Buffalo, NY 14212, phone 716-854-2370; and Glenwood Community Residence at 179 Glenwood Avenue, Buffalo, NY 14212, phone 716-883-6994.
