About this resource
Offers both primary and specialized healthcare, including care coordination, individual and group therapy, health education, training for self-management, and support for treatment adherence for adolescents aged 13 to 24. The team focuses on assisting youth and young adults living with HIV or at risk for HIV, sexually transmitted infections (STIs), those facing homelessness, and individuals dealing with mental health or substance use issues. Additionally, comprehensive support services are available for LGBTQ-identifying youth.
Intake Process: Schedule an appointment via phone, walk-in, or through referrals.
Fees: Services are available to both insured and uninsured individuals, with on-site assistance for insurance eligibility.
