Delivers family-centered care management services, which encompass Comprehensive Care Management, Care Coordination, Health Promotion, Comprehensive Transitional Care, Individual and Family Support, and referrals to community and social support services.

Fees: No charges apply.
Who can use this
- Eligible are children with active Medicaid who have two chronic conditions or a serious emotional disability (SED). The service area includes Erie and Niagara Counties.
