About this resource
This program provides a variety of living arrangements tailored for individuals across all age groups who experience mild to severe developmental delays, multiple physical disabilities, specialized medical requirements, dual diagnoses, or dementia. Each resident receives a personalized service plan that may encompass Intensive Medical Services, Nursing Supports, Therapy Services, Person-Centered Planning and Supports, Behavioral Supports, Wellness and Nutrition Training, and Dementia/Memory Care.
