This service provides round-trip medical transportation for Medicaid recipients in Essex County who do not have alternative means to reach their medical appointments.

Intake Procedure: When requesting transportation, you will need the following information: Medicaid number (e.g., AB12345C), your name, date of birth, address, contact number, reason for transportation, required level of transportation, the name of the ordering physician and the physician you will be visiting, the date and time of your appointment, and the complete address of the medical facility (this is crucial!). If you have a preferred transportation vendor, please include that as well as any additional special instructions for your trip.
