About this resource
This service takes the place of the Targeted Case Management Services. The agency collaborates with Adirondack Health Institute (AHI) to provide connections and support for individuals requiring assistance with their mental and physical health care. Funding is provided by OMH, and there are contracts with Health Home and DOH.
