CCFC provides Health Home Case Management and Home and Community Based Waiver Services. Our care management initiative aids adult individuals in identifying and obtaining the necessary services and supports to tackle their challenges or enhance their quality of life and functioning levels.
Intake Procedure: To schedule an appointment, please call; referrals can be submitted by any agency.
