House Of The Good Shepherd

This program delivers Health Home care coordination services encompassing extensive care management, health promotion, transitional services, and support for individuals and families, along with referrals for community and social support. Each eligible participant is assigned a care manager who serves as the main point of contact for all aspects of the client's care. The care manager advocates for the client and collaborates with the care team to achieve goals that enhance the client's overall well-being—mental, physical, behavioral, and social.

As the program supervisor and intake coordinator, Kurt will assist families in assessing eligibility and the potential benefits of Health Home services.
