About this resource
This program supports families with children aged birth to 3 years who have special needs by facilitating early identification and connecting them to community services.
Intake Process: Referrals are accepted from parents or legal guardians, relatives, healthcare providers, childcare professionals (with parental consent), and community organizations.
Cost: There are no fees involved.
Who can use this
- Families with children from birth to 3 years old whose child meets any of the following criteria: born after a pregnancy of fewer than 33 weeks, weighs under 3 pounds 4 ounces, spent 10 or more days in a neonatal or special care unit, born to mothers younger than 16 years, diagnosed with specific medical issues at or shortly after birth (like heart and respiratory conditions), or identified with special needs and/or potential developmental delays.
