About this resource
This program offers case management services to individuals, facilitating connections for Medicaid HARP-enrolled clients with various support systems including employment, financial assistance, vocational training, and education. The services are tailored to the individual and delivered in the comfort of their own home. Additionally, PEER Empowerment services are available.
Intake Procedure: Clients or providers can reach out via phone or email to obtain information and a referral form.
Fees: Services are provided at no cost.
Who can use this
- Clients must be enrolled in Medicaid and possess an H1 or H9 code.
