This service provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to deliver and supervise medical care at home. The range of services includes medication reminders, grooming and oral hygiene, bathing and dressing assistance, help with mobility, meal preparation, laundry, housekeeping, running errands, and companionship. Additionally, it offers a comprehensive suite of services under the TBI/NHTD Waiver, which encompasses Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. Non-medical transportation services are also available. Care management is provided through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY), along with tuition-free certification for Personal Care and Home Health Aides. Fees are covered by NHTD and TBI Medicaid Waivers, CDPAP, MLTCs, MCOs, long-term care insurance, and private pay options, but sliding scale payments are not available.
