This program facilitates communication among healthcare providers and provides assistance during transitions of care. Services offered include health promotion, support for patients and their families, and referrals to community and social services.

- Individuals must be 18 years or older, have chronic health conditions (either physical or behavioral), and be eligible for Medicaid.
