About this resource
This program provides access to various support services from the New York State Department of Health, such as the Children's Health Home, which focuses on care management for chronic medical, mental health, substance abuse, behavioral health, and developmental requirements. Additionally, the Consumer Directed Personal Assistance (CDPAP) is a Medicaid service enabling individuals with chronic conditions, with or without developmental disabilities, to employ and train caregivers for their assistance needs.

Who can use this
- Individuals must be New York State residents eligible for Medicaid, elderly, disabled, or in need of assistance with daily activities or skilled care, and should be medically stable with the ability to manage their own care or have a designated representative.
