Facilitates the integration of various care types that a participant might require, including behavioral health, medical, and social services.
Intake Process: A referral must be completed via HHUNY or GRHHN.
Costs: Services are provided at no charge.
Who can use this
- Individuals eligible for Medicaid, including those already part of a Managed Care Organization (those with both Medicaid and Medicare also qualify), must meet one of the following criteria: having two or more chronic health issues, such as asthma, diabetes, heart disease, a significant mental health condition, or a substance use disorder, or a diagnosis of HIV/AIDS.
