About this resource
This program offers a range of services including Comprehensive Care Management, Care Coordination, Health Promotion, Comprehensive Transitional Care, Individual and Family Support, and referrals to community and social support resources.
Intake Process: Fill out the Children's Health Home of Upstate New York (CHHUNY) referral form. Referrals may be sent to CHHUNY or directly to Hillside.
Costs: There are no service fees.
Who can use this
- Eligible participants are children aged 0 to 21 with active Medicaid and either two chronic conditions or one of the specified qualifying conditions: HIV/AIDS, Serious Emotional Disturbance (SED), or Complex Trauma.
