This service aids families caring for individuals with Alzheimer's disease and other dementia types by offering caregiver consultations and assistance with navigating health care services, education and training, long-term care options, planning, respite care, support groups, and wellness programs.

- Caregivers of individuals with Alzheimer's or related dementia, as well as those with mild Alzheimer's or memory loss who can engage with a navigator.
