This service pairs individuals with a care manager to formulate a personalized plan to address their medical requirements, which may include care management, home health aides, medication management, medical equipment, physical therapy, speech therapy, respiratory therapy, occupational therapy, and more.

Fees: Covered by Medicaid.
Who can use this
- Individuals aged 18 and above who qualify for nursing home-level care, can safely remain at home, require long-term care for at least 120 days, are eligible for Medicaid, and reside in the service area.
