This program offers care management services to families who are part of a Children's Health Home. It is important to note that a Children's Health Home is not a physical facility but a virtual support network that acts as a central point of contact, coordinating services and delivering compassionate care.
Intake Procedure: To initiate the application process, reach out to Admissions.
Fees: The service is covered by Medicaid, and the Care Manager can also connect families to HCBS waiver or waiver services.
Who can use this
- Children and young adults aged 0 to 21 who are enrolled in Medicaid and have two chronic health conditions (such as HIV/AIDS, Serious Emotional Disturbance, Complex Trauma, or other disabilities) are eligible.
