About this resource
This initiative assists individuals with intellectual and developmental disabilities, along with their families, in navigating available services and supports. It encourages informed choices and fosters independent decision-making. Care Coordinators are dedicated to linking individuals with essential care, support, and opportunities, while also monitoring their needs and services during life transitions. They facilitate access to medical, dental, and behavioral health services, ensure safety and residential requirements are met, and provide referrals to community and social support services. Additionally, they develop a comprehensive Life Plan that evolves with the individual's goals and needs.

Fees: No charge for those eligible through OPWDD and Medicaid navigation, with Medicaid being a requirement for enrollment.
Who can use this
- Individuals diagnosed with intellectual and/or developmental disabilities before the age of 22, along with significant impairments in social, communication, and daily living skills, as well as conditions like autism spectrum disorders, cerebral palsy, epilepsy, neurological impairments, familial dysautonomia, and Prader-Willi syndrome, qualify for this program.
