About this resource
This program delivers care management and support services aimed at helping you achieve your personal health objectives. It includes coordinating transportation for medical appointments, understanding complex medical needs, and connecting you with necessary care. Additionally, it offers assistance with insurance options such as ACA Marketplace, ADAP, private insurance, Medicaid, Medicaid spend-down, and Medicare Part D. The service also involves attending medical and pharmacy visits for medication follow-ups, finding primary care and other medical services, coordinating mental health and substance abuse referrals, and exploring additional services like nutrition education, day health programs, and food pantry access. Housing assistance and LGBTQ-focused care management services are also available.

Fees: Case management is billed to Medicaid or provided at no cost. Other services are offered free of charge.
