About this resource
Provides Medicaid Service Coordination that includes advocacy by connecting individuals with intellectual and developmental disabilities to community supports and services, as well as creating and revising individualized service plans in collaboration with the person, their family, and the support team. Additionally, it offers community habilitation and educational resources.
Intake Procedure: Clients are encouraged to contact the agency for further information.
Fees: The program is funded through Medicaid.

- Eligible individuals are adults with intellectual or developmental disabilities living in Nassau or Suffolk County.
