We partner with you to gather a comprehensive medical history and encourage you to inform us about any external care you receive. We may inquire about your current medications, family medical history, and any recent hospital visits. Our aim is to communicate clearly and provide timely appointments. We utilize the latest medical research to inform our discussions about your care options. To assist you in managing your health at home, we offer tools such as log books and reminder calendars. We are dedicated to building a strong relationship with you and equipping you with the necessary services and resources to maintain your well-being. We accept Medicaid, Medicare, and most commercial insurances, and for those without insurance, enrollment counselors are available to help. A sliding-fee scale is offered for individuals who do not qualify for insurance. Co-payments and deductibles must be settled at the time of service. Our services encompass chronic condition management, referrals to specialists, care coordination, routine annual checkups, pre-employment physicals, lab work, CDL and DSS physicals, as well as pre-operative evaluations.
