We operate five Family Health Centers that deliver crucial primary care services and act as 'medical homes' for residents in some of Long Island's most neglected areas. By offering high-quality primary care and an expanding range of specialty services directly within the community, we aim to ensure that more Long Islanders receive necessary care where it is most accessible. Our focus is on maintaining the health of our patients to prevent hospitalizations. Through Harmony Healthcare Long Island (HHLI), we are introducing innovative health services to the communities that require them the most.
