Tuscarora Health Center - Nfmmc

This health center delivers a wide array of services, such as primary care (including assessment, treatment, and referrals), a dental clinic, health education, and diabetes management. The Tuscarora Community Health Worker Program offers home-based case management for enrolled women and their children, helping them navigate barriers to healthcare access. Community workers identify challenges that hinder clients from receiving necessary care, assist pregnant women in obtaining prenatal services and children in accessing primary care, and help with enrollment for programs like WIC, Medicaid/PCAP, and Child Health Plus. They also provide education on various topics including breastfeeding, baby care, lead poisoning, immunizations, nutrition, HIV/AIDS, domestic violence, and substance abuse, while offering referrals to food pantries, housing assistance, educational programs, childcare, and additional services. The goal is to support families in achieving a healthy start. Operating hours are Monday from 9am to 6pm and Tuesday to Friday from 9am to 5pm. The Tuscarora Community Health Workers Program operates Monday to Friday from 8:30am to 4pm.
